Lakeview Middle School and the Calhoun County Public Health Department are pleased to announce a new service for our students, the Lakeview Health Center. The Lakeview Health Center will be housed within Lakeview Middle School and will offer many opportunities for students at Lakeview Middle and High Schools and Calhoun Community High School.
 
What services will be provided?
Students may be treated for minor illnesses and injuries. They may obtain immunizations, yearly physical exams, mental health counseling, and general health education. Referrals and follow-up for more serious and chronic illnesses will be handled on an individual basis. Minors may legally seek advice, testing and treatment for drug abuse, substance abuse, HIV, sexually transmitted diseases, and pregnancy.
 
What services will NOT be provided?
According to state law, the clinic will not provide birth control pills, other forms of contraception, or abortion counseling.
 
Who staffs the clinic?

A family nurse practitioner, medical assistant, and social worker will provide services.

 
How do you take advantage of this new service?

  1. Complete the “Consent for Treatment” form.
  2. The four-page consent form must be signed by the child’s parent or legal guardian. Please use black or blue ink only.
  3. The form must be returned to the school office before requesting treatment.

 
What are the costs of this service?
No child will be denied services for inability to pay, however
  1. If you have insurance, services will be billed to your provider. Please provide a copy of the front and back of your insurance card.
  2. If you do not have insurance, applications for insurance programs are available at the health center or you can visit www.michigan.gov/mibridges. A sliding fee scale based on the student’s income will be used to determine charges for services provided to uninsured students.
  3. No student will be charged at time of service.
